Add Pages to
Existing Passport
You may use Application for Additional Visa Pages (DS-4085)
if you wish to add visa pages to your existing U.S. passport.
Please send us the following documents:
- Your current U.S. passport. (Passport MUST be signed.)
- Application for Additional Visa Pages (DS-4085), fully completed and signed by applicant.
- Print and fill out our Letter of Authorization, signed and dated by applicant. This letter will allow us to submit and pick up your completed passport on your behalf. Please note that the signature MUST match the signature in the passport. Also, signatures MUST be original, no copies or faxes.
- For the processing time of less than two weeks, you will need to provide an itinerary or copy of your air tickets that reflect a destination outside of the U.S. If you are requesting same or next day processing, you must show a departure date within 5 business days from date of your application. (Please contact us if you are requesting a passport amendment to be processed same or next day for complete details.)
- Print and fill out our Passport Processing Order Form.
