You may use U.S. Passport Re-application Form (DS-5504) if you wish change your name on your existing passport due to marriage or court order WITHIN one year from the date of issuance of passport.

Please send us the following documents:

1. Your original valid U.S. passport. (Passport MUST be signed.)
   
2. U.S. Passport Re-application Form (DS-5504), fully completed and signed by applicant.
3. Two identical 2" x 2" passport type photos with plain white background. Photos must be in color and of good quality.
4. If your current passport has been more than a year old from date of issue, you must submit a Passport Renewal Form (DS-82) and pay the government fee.
5. Print and fill out our Letter of Authorization, signed and dated by applicant. This letter will allow us to submit and pick up your completed passport on your behalf. Please note that the signature MUST match the signature in the passport. Also, signature MUST be original, no copies or faxes.
6. For the processing time of less than two weeks, you will need to provide an itinerary or copy of your air tickets that reflect a destination outside of the U.S. If you are requesting same or next day processing, you must show a departure date within 5 business days from date of your application. (Please contact us if you are requesting a passport amendment to be processed same or next day for complete details.)
7. Original document reflecting the new name in which you would like your passport to read, i.e. Marriage certificate, divorce decree, adoption papers, etc. The U.S. State Department will return your original documentation back to you along with your amended passport.
8. Print and fill out our Passport Processing Order Form completely.
