First Time or Renewal
for Minors Under 16
If you are applying for a first time passport or a renewal for a
minor under the age of 16, you must follow the instructions below.
BOTH parents or legal guardian must bring the following documents
to an Acceptance Agent and have them sealed:
- U.S. Passport Application Form, fully completed.
DO NOT SIGN THIS FORM DS-11 UNTIL ACCEPTANCE AGENT ASKS YOU TO.
DO NOT fill in the address where the passport is to be mailed - leave that blank.
- ORIGINAL proof of U.S. citizenship.
- If you are renewing a passport for a minor, the minor's original birth certificate must be provided and sealed.
- Two recent 2" x 2" identical photographs of the minor with a plain white background and taken less than 3 months ago.
- For the processing time of less than two weeks, you will need to provide an itinerary or copy of your air tickets that reflect a destination outside of the U.S. If you are requesting extreme expedited processing, you must show a departure date within 57 business days from date of your application. (Please contact us if you are requesting the minor's passport to be processed same or next day for complete details.)
- If the second parent or legal guardian is not available to sign, he/she will need to fill out a Statement of Consent: Issuance of a Passport to a Minor Under Age 14 (DS-3053). This form must be NORTARIZED and should be submitted along with DS-11.
- $112.00 in check or money order payable to "U.S. Department of State". Please note that starting check will not be accepted. You will need to pay $30.00 execution fee to Acceptance Agent.
Print and fill out TWO Letters of Authorization, place one inside the envelope and attach one OUTSIDE your sealed envelope. This will allow us to see the name of the applicant inside, along with the date of birth and date of departure.
NOTE: Currently, minors MUST appear in person with BOTH parents. |
