Passport Renewal,
Age 16 & Older
You may use Application for U.S. Passport by Mail (DS-82) if you:
- Can submit your most recent U.S. passport.
- Were at least 16 years old when your most recent U.S. passport was issued.
- Were issued your most recent U.S. passport less than 15 years ago.
- Were issued your most recent U.S. passport that was valid for 10 years
- Are using the same name on your most recent U.S. passport; or havehad your name changed by marriage or court order and can submit proper documentation to reflect your name change.
Please send us the following documents:
- Your current or most recent passport - can not be expired for more than 5 years. (Passport MUST be signed.)
- Application for U.S. passport by Mail (DS-82) must be completed and printed out from the U.S. Department of State web site, Application for U.S. Passport by Mail. Failure to do so will significantly delay your application. After entering the information, click "Create Form". Print out and sign the completed application Form.
- Two Letters of Authorization.
- Passport Processing Order Form.
- Two 2" x 2" identical color photographs with a plain white background and taken less than 6 months ago.
- For the processing time of less than two weeks, you will need to provide an itinerary or copy of your air tickets that reflect a destination outside of the U.S. If you are requesting same or next day processing, you must show a departure date within 5 business days from date of your application.
